Post-traumatic stress disorder (PTSD) remains one of the most challenging mental health conditions to treat. While psychotherapy and medication can provide meaningful benefits for many people, a significant proportion of individuals continue to experience persistent symptoms that affect their daily lives, relationships, and wellbeing.
In the latest episode of the neurocare Academy Podcast, Dr. Trevor Brown sat down with Dr. Noah Philip, Professor in the Department of Psychiatry and Human Behavior at Brown University, to discuss how neuromodulation technologies are reshaping the treatment landscape for PTSD and other mental health conditions.
From the growing evidence supporting transcranial magnetic stimulation (TMS) for PTSD to the future potential of focused ultrasound, the conversation highlights how advances in neuroscience are creating new opportunities for more personalized and effective care.
The Evolution of TMS for PTSD
When TMS first emerged as a treatment for depression, patients with PTSD were frequently excluded from clinical trials. As a result, PTSD was often viewed as a condition outside the scope of neuromodulation treatment.
Over the past decade, however, that picture has changed considerably.
According to Dr. Philip, clinicians began observing that people receiving TMS for depression often experienced improvements in PTSD symptoms as well. These real-world observations were reinforced by emerging neuroimaging research showing that brain networks targeted by TMS in depression patients are also involved in PTSD-related dysfunction.
Today, evidence from numerous clinical studies suggests that TMS can be an effective treatment option for patients with PTSD, producing response rates that are increasingly comparable to those achieved in major depressive disorder.
This represents a significant shift in the field and offers new hope for patients whose symptoms have not responded adequately to conventional approaches.
A Familiar Tool with Broader Applications
One of the most practical messages from the discussion is that clinicians who already provide TMS for depression may be closer to treating PTSD than they realize.
As Dr. Philip explains, many patients receiving TMS for depression have co-occurring trauma histories or PTSD symptoms. In many cases, clinicians have already been helping these individuals through standard treatment protocols.
Importantly, current evidence suggests that treating PTSD does not require entirely new equipment, specialised protocols, or extensive additional training beyond standard TMS competencies. Existing approaches targeting the dorsolateral prefrontal cortex have demonstrated encouraging results in PTSD populations across multiple studies.
While ongoing research continues to explore optimization strategies, the ability to implement PTSD-focused care using existing clinical infrastructure could help expand access to evidence-based treatment more rapidly.
Looking Beyond Diagnosis to Brain Networks
A recurring theme throughout the interview was precision medicine.
Rather than focusing solely on diagnostic categories, researchers are increasingly examining the underlying brain networks involved in psychiatric symptoms. PTSD is characterized by disruptions in neural circuits involved in threat detection, emotional regulation, memory processing, and salience attribution.
Research conducted by Dr. Philip and colleagues suggests that successful TMS treatment may help normalize these network-level abnormalities, reducing maladaptive connectivity patterns while strengthening connections associated with healthy emotional regulation.
The ultimate goal is to move beyond asking whether a treatment works for PTSD in general and instead identify which intervention is most likely to benefit a particular patient based on their unique neurobiology.
While imaging-guided treatment selection is not yet standard clinical practice, advances in brain connectivity mapping continue to bring the field closer to truly personalized psychiatry.
Accelerating Recovery
Another exciting area of development discussed during the interview is accelerated TMS.
Traditionally, TMS treatment courses require daily sessions over several weeks. Accelerated protocols condense treatment into a much shorter timeframe by delivering multiple stimulation sessions per day.
Early findings from Dr. Philip's work suggest that accelerated approaches can achieve promising response and remission rates in PTSD populations, while maintaining practical feasibility in real-world healthcare settings.
Although additional randomized controlled trials are still underway, these developments support a future where patients may be able to access effective treatment more quickly than ever before.
The Next Frontier: Focused Ultrasound
Looking further ahead, Dr. Philip discussed one of the most innovative areas of neuromodulation research: low-intensity focused ultrasound.
Unlike TMS, which primarily affects cortical brain regions near the surface, focused ultrasound has the potential to modulate deep brain structures non-invasively. This could enable researchers to directly influence targets such as the amygdala, a key structure involved in fear processing and emotional learning.
However, Dr. Philip was careful to emphasize that this technology remains experimental. Research is ongoing to better understand safety, dosing, target engagement, and clinical effectiveness before widespread clinical implementation can be considered.
At the same time, the potential is significant.
If successful, focused ultrasound could offer a way to influence brain circuits that are currently difficult to reach using non-invasive technologies. For PTSD and many other psychiatric conditions, this may open entirely new treatment possibilities.
Looking Ahead
For Dr. Philip, the future of mental healthcare is increasingly clear: faster interventions, more personalized treatment selection, and technologies that directly target the neural circuits involved in suffering.
While many questions remain, the progress made over the past two decades has been remarkable. Neuromodulation has evolved from a niche area of psychiatry into a rapidly advancing field capable of delivering meaningful improvements for patients who previously had limited options.
As research continues to advance, the vision of precision neuromodulation - matching the right intervention to the right patient at the right time - moves ever closer to reality.
